Job Summary The essential job functions for this position include, but may not be limited to those listed in this job description. This position requires knowledge of disease management principles and the use of a registry for population based management of medical conditions. The case management coordinator will oversee our disease management programs for high risk patients including but not limiting diabetes, hypertension, cardiovascular disease, chronic kidney disease, cancer screening and prevention, patients during transition of care such as post ER, hospitalization, pregnancy, and other conditions, utilizing medical assistants to provide phone contacts and reminders to providers and patients per protocol. The case management coordinator will also be required to order appropriate tests, immunizations and referrals by protocol and provide health education counseling and classes to patients with chronic disease and prenatal patients. II. Essential Functions Efficiently and accurately follows Chronic Disease Care Management protocols, including ordering appropriate tests, referrals and immunizations. Demonstrates ability to make independent clinical decisions in routine patient care matters. Able to understand and follow a risk management protocol- assists the agency in mitigating risk of adverse events. Promotes the level of clinical expertise required of staff to provide safe, high quality of nursing care. Remains flexible and responsive when changes occur in patient activity and workload. Performs appropriate documentation to maintain the standards set by HCHC and nursing practice. Develops, organizes and plans the execution of health educational programs based on our Health Care Guidelines to promote Health Maintenance, prevention of chronic disease and prevention of complications of Chronic Diseases. Develops and conducts health education/promotional presentations of health related topics. Fosters an environment for open communication and collaborative practice. Demonstrates knowledge of therapeutic action, side effects and interaction of medications. Acts as a comprehensive care coordinator for the care of assigned patients, assessing patients' needs accordingly and facilitates communication amongst other providers. Performs comprehensive assessment of physical, emotional, psychosocial, and environmental needs for patient. Assumes responsibility for maintaining compliance according to federal and state regulations and agency policies and procedures. Identify significant clinical findings, draws conclusions and intervenes appropriately. Participates in development of patients' goals and discharge plans as well as makes revisions to these based on the changes in patient status. Implements Provider's orders accurately and promptly. Maintains confidentiality of all agency and patient information according to HIPAA guidelines. Communicates information effectively both verbally and in writing. Organizes workload to complete responsibilities in an appropriate and timely manner. Help lead and incorporate technology (EMR), innovation, and customer service into patient care protocols. Utilizes material, equipment and time in a safe, beneficial and cost effective manner. Observe infection control and universal precautions. Strives to provide quality of services to a diverse ethnic groups. Management of medical records before they are released per requests. Assists in monitoring clinical measures. Participates in health education activities. Performs other duties as assigned. III. Organization Expectation Adhere to all HCHC's Policies and Procedures. Able to work independently and also in a team manner. Attend staff meetings and complete all assigned trainings. IV.
Education/License/Certification:
Three years of clinical or case management experience preferred. Current CPR certification required. V. Skills and Specifications Experience in the field related to health car. Experience as Case Management Coordinator preferred. Knowledge of regulations governing community clinics. Experience in developing and implementing educational programs. Good interpersonal skills. Team oriented management style. Excellent communication skills required. Able to work flexible hours including weekends. Fluent in Bilingual English and Cantonese/Mandarin/Vietnamese/Spanish preferred. VI. Physical Requirement Must be able to complete all physical requirements of the job with or without a reasonable accommodation. Must be able to remain in a stationary position 50% of the time. May need to move boxes weighing up to 25 pounds across the office for various needs occasionally.
