Job Description

At NorthBay Health the Transfer Center Coordinator is a critical role in the NorthBay Health system facilitating both inbound and outbound transfers. The Transfer Center Coordinator is responsible for facilitating and coordinating patient transfers both inbound/outbound, tertiary transfers both emergent and urgent, consults from NorthBay Health to tertiary acute care hospitals. Sets up systems to enable both cost containment and quality patient care. This position requires excellent communication and organizational skills to ensure seamless coordination between various departments, healthcare facilities, and external agencies. The Transfer Center Coordinator works collaboratively with all areas of NorthBay Health to facilitate timely and appropriate transfer of patients regardless of complaint or need. This position will require liaison with the case management/utilization review team as well as the Administrative Coordinator. Areas of focus are capitated patients, repatriations, emergent and urgent transfers, notification calls to health plans, re-triage of pediatric/trauma/stroke/stemi patients. The Transfer Center Coordinator acts as a liaison between the medical staff, nursing staff, pre-hospital personnel, referring centers, and external EMS agencies.
Education :
Associate Degree in a healthcare related field required Bachelor's degree preferred
Experience:
Minimum two years of related clinical experience. Previous experience coordinating transfer services preferred. Excellent oral and written communication. Must be well organized, able to set priorities, work independently and serve as an effective team member. Word Processing skills required. Statistical/data management skills preferred. Ability to work efficiently in a fast-paced and high-pressure environment. Proficiency in healthcare IT systems and basic computer software.

At least four (4) shifts each month, or more as determined by the department's per diem availability standard and process
Shift :
10-hour shifts 10% per diem differential included in salary range Plus, Generous Shift Differentials
