Blize Healthcare. Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator to join our team. This role is essential in ensuring patients experience a smooth, supportive transition into home health and hospice care.
Responsibilities:
Manage daily patient referral and intake operations. Verify insurance coverage and assist the billing department with claim processing. Complete insurance forms and respond to patient inquiries. Schedule home health and hospice visits efficiently. Answer, screen, and direct phone calls; take messages; schedule appointments. Greet and assist visitors in a professional manner. Receive, sort, and distribute mail, documents, and packages. Maintain medical supply inventory. Select and package DME supplies. Support colleagues with general administrative tasks. Maintain Patient Record. Send Orders needed to sign to physicians and follow up with physicians. Attach patient records and consent forms in the company system.
Requirements:
Minimum 1 year of experience in the medical field. Strong verbal communication skills. Excellent interpersonal and customer service skills. Basic understanding of administrative and clerical procedures. Proficiency in Microsoft Office (Word, Excel, Outlook) and Google Workspace (Docs, Sheets).
