JOB SUMMARY
Assists Solheim Senior Community ("Solheim") in providing exceptional resident-centered care and customer service by coordinating communication between residents, families, nursing staff, physicians, and other departments. Serves as the primary point of contact for residents and visitors while supporting resident care coordination. Ensures resident needs are addressed promptly, professionally, and in compliance with all Federal, State, and local regulations.
ESSENTIAL JOB FUNCTIONS
Serve as a liaison between residents, families, nursing staff, physicians, and community departments. Assist residents and families by answering questions, resolving concerns, and directing requests to the appropriate staff member. Coordinate appointments, transportation requests, physician visits, therapy schedules, and follow-up services as needed. Monitor resident requests and ensure timely communication and resolution. Support nursing staff by communicating resident condition changes, concerns, and service needs. Assist with admission, transfer, and discharge processes, including preparation of required documentation. Maintain resident demographic information, emergency contacts, and census records. Ensure resident rights, dignity, privacy, and confidentiality are maintained at all times. Greet residents, families, guests, vendors, and visitors in a professional and welcoming manner. Answer and route incoming calls promptly and accurately. Provide information, directions, and assistance to residents, visitors, and staff. Enter and maintain resident information within electronic systems. Process resident-related paperwork, forms, and departmental documentation. Maintain compliance with HIPAA, Solheim policies, and applicable regulatory requirements. Report resident concerns, safety issues, incidents, or unusual occurrences to the appropriate supervisor immediately. Participate in emergency preparedness activities, fire drills, and safety training. Assist departments in preparation for surveys, inspections, and audits. Maintain regular and consistent attendance. Perform other duties as assigned.
POSITION REQUIREMENTS
EDUCATION/EXPERIENCE:
High school graduate or equivalent. Current CNA certificate Prefer experience with Dementia/Alzheimer's care Minimum two (2) years of experience in healthcare, senior living, medical office, customer service, or administrative support preferred.
SKILLS/KNOWLEDGE/ABILITY
Desire and ability to work with ill, aged, and disabled persons. Willingness and ability to read, write, speak, understand English and have the communications skills necessary to provide accurate information to residents and staff. Willingness and ability to follow written and verbal direction in English. Willingness and ability to maintain appropriate level of confidentiality and privacy. Willingness and ability to interact professionally with all customers, including the ill, aged, disabled persons and co-workers, individually and as part of a team. Willingness and ability to effectively handle multiple items/tasks as required and adapt favorably to changing priorities. Willingness and ability to make appropriate judgments, decisions and problem solving in a timely manner and within the context of the situation at hand. Ability to effectively prioritize items/tasks as required. Willingness and ability to effectively respond to emergency and/or crisis situations. Willingness and ability to take initiative and be a self-starter. Willingness and ability to understand and comply with Federal, State, and local regulations.
