Sequoia Home Health is growing and looking for experienced, compassionate Home Health Clinical Manager to join our team! Job Summary The Clinical Manager oversees home health clinical operations, ensuring high-quality patient care, regulatory compliance, and effective care coordination. This role leads an interdisciplinary team and ensures services align with physician orders and individualized care plans. Key Responsibilities Manage clinical staff and daily operations in a home health setting Develop and oversee plans of care (POC) and patient outcomes Review referrals and assign clinicians based on patient needs Ensure compliance with CMS, Medicare, and agency standards Conduct chart audits, case reviews, and case conferences Collaborate with physicians and support care coordination Participate in QAPI and performance improvement programs Assist with hiring, training, and staff development Qualifications Licensed RN, PT, OT, SLP, or MSW (active state license) 2+ years home health experience required Supervisory or leadership experience preferred Strong knowledge of home health regulations & documentation What makes us unique ?
