Job Description
  • Coordinate team based medical care for the children placed in the center.
  • Prepare, compile, submit, and maintain accurate medical records, files, forms, statistics, and additional relevant information in accord with agency policy, licensing and/or funding requirements.
  • Review ORR portal for compliance with medical information upload requirements and ensure updates are input as needed for safety and timely discharge.
  • As appropriate, provide sponsors, family members and clients with medial updates.
  • Actively participate in medical and service-related resource development prior to children's discharge.
  • Participate in planning, coordinating, implementing, and evaluating all medical needs of the children for designated social service program(s).
  • Establish and maintain partnership with medical care providers.
  • Provide documented information related to identified delays in the medical assessments.
  • Attend scheduled meetings and participate in team decisions and operations to represent the medical needs of the children.
  • Coordinate medical resources for all employees and resource family homes Adhere to Child Abuse and reporting requirements.

  • 2+ years working experience in child welfare.
  • 1+ years of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
  • Must be at least 21 years of age and have a
Valid Driver License Education/ Experience:
Licensed Registered Nurse (RN) with a bachelor's degree or a higher health-related qualification
Language Ability:
Bilingual in Spanish requested but not required.
