This is a full-time benefited position working 10.25-hour shifts on various days of the week including weekend and holiday rotation. Position typically starts at 7/7:30am and ends at 5:45/6:15pm. We invite you to join our Scripps Mercy San Diego Case Management team as an Inpatient Complex Care Manager. As an Inpatient Complex Care Manager, you will be responsible for the daily management and coordination of care for patients in the hospital with Identified complex health needs, including coordination with providers, navigators and ancillary and community resources across the system and care continuum. Additionally, you will: Has knowledge of involuntary hold or detention status (to danger to self, danger to others or gravely disabled), conservatorships, public and private as well as Mental Health care needs. Has extensive knowledge regarding multiple insurance plans operating within the service area and may assist patients in coordinating change in insurance coverages and/or assisting to determine if additional medical insurance coverage or government funding is available. Assists in coordination and management of patients outside of the hospital for which hospitals may need to follow due to contractual or transition planning post discharge.
