$21.50 - $23.00 per hour Position Summary Buena Vista Care Center is seeking a compassionate, detail-oriented, and highly organized Patient Liaison Coordinator to serve as a key connection between patients, families, and the care team within our skilled nursing facility. This role is essential in enhancing the patient experience by providing consistent communication, administrative coordination, and ongoing support throughout each patient's stay.

The Patient Liaison Coordinator will play a vital role in welcoming new admissions, coordinating appointments, managing patient flow, assisting with insurance and billing processes, and conducting routine check-ins to ensure patient satisfaction, comfort, and overall well-being. Key ResponsibilitiesPatient Experience & Liaison Support Serve as the primary point of contact for patients and families regarding non-clinical needs and general inquiries Greet and orient patients upon admission to ensure they feel welcomed, informed, and comfortable Conduct routine patient check-ins to assess satisfaction, address concerns, and provide ongoing support Educate patients and families on facility services, schedules, and available resources Communicate patient feedback, concerns, or needs to appropriate departments promptly Build and maintain positive relationships with patients, families, and staff to enhance overall patient experience Administrative Coordination & Scheduling Coordinate and schedule patient appointments, including internal services and external medical visits Manage patient flow throughout the facility Process patient check-in and check-out procedures accurately and efficiently Provide administrative coordination related to admissions and ongoing care services Insurance, Billing & Authorization Support Verify patient insurance coverage Assist with insurance and billing documentation Obtain prior authorizations for procedures and services as required Coordinate with business office and administrative teams to ensure accurate processing Documentation & Records Management Maintain, update, and organize patient records, care planning documents, and related data Document patient interactions, concerns, and follow-up actions in a timely manner Ensure accuracy, confidentiality, and compliance when managing sensitive patient and insurance information Team Collaboration Support the interdisciplinary care team in promoting patient-centered care and service excellence Assist with issue resolution to support continuity of care and patient satisfaction Qualifications High school diploma or equivalent required Additional education in healthcare administration, communications, or a related field preferred Previous experience in healthcare, customer service, medical office, or skilled nursing facility preferred Strong interpersonal, communication, and organizational skills Ability to multitask, prioritize, and manage time effectively in a fast-paced environment Compassionate, professional, and patient-focused approach Ability to handle confidential information with discretion and integrity Work Environment Fast-paced skilled nursing facility setting Frequent interaction with patients, families, and healthcare staff Requires regular walking throughout the facility and direct patient engagement Why Join Us?
