Referral Coordinator, Full-Time Rural Health Clinic | Arkansas Valley Regional Medical Center (AVRMC) Help patients navigate their care journey with coordination, compassion, and attention to detail. We are seeking a detail‑oriented, organized, and compassionate Referral Coordinator to join our team in La Junta, Colorado. Located just east of the Rocky Mountains, AVRMC proudly serves Southeastern Colorado by providing high‑quality healthcare close to home. Position Summary As a Referral Coordinator, you will coordinate specialty care for patients requiring external consultations as directed by primary care providers. You will manage referrals from start to finish, including scheduling, authorization, tracking, and follow‑up, while ensuring a positive, professional patient experience. Key Responsibilities Patient Coordination & Support Provide excellent patient service both in person and over the phone with a warm, professional demeanor Review referral details with patients and set clear expectations for next steps Serve as a resource for patients and providers regarding referral questions or concerns Maintain patient confidentiality and respect patient rights at all times Referral Processing & Documentation Process referrals by preparing medical records and forwarding appropriate clinical information to specialists Obtain and verify accurate patient demographic and insurance information Secure authorizations for referrals, diagnostic testing, and procedures; notify physicians of denials Maintain ongoing tracking of referrals through logs and/or Electronic Health Record (EHR) documentation Ensure referrals are completed accurately and in a timely manner, prioritizing urgent needs Scheduling & Coordination Schedule patient appointments with specialty providers Coordinate communication between physicians, specialists, and departments to support continuity of care Build and maintain strong relationships with specialty providers and community resources Team Support & Flexibility Assist front office staff with administrative duties when needed Assist back-office staff with Medical Assistant duties when applicable Maintain flexibility and support overall clinic operations as needed Professionalism & Quality Maintain consistent attendance, punctuality, and professional appearance Adhere to all health, safety, and organizational policies Demonstrate performance aligned with AVRMC's mission, values, and standards Perform additional duties as assigned Qualifications Required High School Diploma or GED equivalent Current CPR Certification Strong computer skills with experience in PC‑based systems Ability to maintain strict patient confidentiality Preferred Spanish language fluency Previous experience in healthcare, referrals, or medical office setting Why Join AVRMC At AVRMC, laboratory staff are an integral part of the patient care team.
