Kim Chainhalt, Assistant Agency Manager Position Summary The Care Manager serves as the primary relationship owner for assigned clients and caregivers. This role is responsible for ensuring an excellent client experience through proactive communication, relationship management, in-home visits, caregiver support, and oversight of all client-related services. The Care Manager works closely with clients, families, caregivers, and internal team members to ensure care is consistent, responsive, personalized, and aligned with the Agency's standards and CARE values. This position emphasizes meaningful client interaction and in-person engagement over administrative coordination. The Care Manager is expected to maintain strong relationships with assigned clients and caregivers, anticipate needs before issues arise, and ensure clients feel known, supported, and cared for throughout their experience with Visiting Angels. Core Focus Areas Client relationship management In-home supervisory visits Caregiver matching and continuity Client satisfaction and retention Proactive communication with families Caregiver coaching and support Service oversight and problem resolution Collaborative scheduling leadership Essential Duties Serves as the primary point of contact and relationship owner for assigned clients and families Develops a strong understanding of each client's care needs, preferences,routines, personality, and goals Conducts regular in-home supervisory and quality assurance visits Builds meaningful relationships with clients through consistent communication and personal interaction Proactively identifies concerns, service changes, risks, or opportunities to improve the client experience Ensures clients and families feel informed, supported, valued, and cared for Collaborates with clients, families, caregivers, and healthcare professionals to support continuity of care Introduces new caregivers to clients and supports successful caregive transitions Responds promptly and compassionately to client concerns, complaints, or service issues Maintains accurate and current client documentation and care plans Caregiver Support & Oversight Coaches and supports caregivers toward professional excellence and alignmentwith Agency expectations Promotes caregiver consistency and successful client/caregiver matches Conducts caregiver supervisory visits and performance feedback Provides ongoing education, encouragement, and support to caregivers Collaborates with HR and leadership regarding employee performance concerns when needed. Scheduling & Service Coordination Oversees scheduling needs and service continuity for assigned clients Collaborates with fellow Care Managers to resolve coverage needs Prioritizes continuity of care and quality client/caregiver matches in scheduling decisions Supports staffing coordination while maintaining focus on client satisfaction and caregiver experience Verifies holiday schedules and communicates care needs proactively with clients and caregivers Assists with urgent staffing or service issues as needed Agency & Professional Responsibilities Demonstrates professionalism, compassion, accountability, resilience, and excellence in all interactions Maintains knowledge Agency policies, HIPAA, and privacy practices Participates in on-call rotations as assigned Participates in training, meetings, and professional development opportunities Supports Agency culture and collaborates positively with office team members Demonstrates flexibility and willingness to support operational needs as necessary Job Qualifications Must be at least 21 years of age with a high school diploma or GED Strong interpersonal and relationship-building skills Excellent verbal and written communication skills Passion for serving seniors, families, and caregivers Ability to remain calm, compassionate, and solutions-focused under pressure Strong organizational and problem-solving abilities Ability to balance client needs, caregiver needs, and operational priorities Comfortable working in a fast-paced and highly relational environment Strong customer service mindset Ability to work independently while collaborating effectively with a team Competent with Axis Care and other scheduling/software systems Valid driver's license, reliable transportation, and proof of insurance required Willingness to spend significant time in the field visiting clients and caregivers Working Environment Full-time, non-exempt position Combination of office and field work responsibilities Frequent driving to client homes and community locations Regular interaction with clients, families, caregivers, referral partners, and healthcare professionals Expected to spend substantial time outside the office conducting client and caregiver visits evening/weekend on-call rotations as assigned Participation in evening/weekend on-call rotations as assigned

$24.00 - $27.00 per hour
