This is a unique opportunity to blend clinical expertise, community outreach, and leadership to improve mental health access and outcomes for communities. What You'll Do Clinical Care & Community Impact Provide comprehensive behavioral health services including psychiatric evaluations, and individual, group, and family therapy Deliver flexible, community-based care tailored to meet clients where they are Collaborate with partner sites to ensure culturally responsive care and help bridge gaps between families, agencies, and community resources Serve as a cultural and clinical bridge for monolingual Spanish-speaking clients Program Development & Leadership Help shape and enhance culturally responsive behavioral health programming Collaborate with clinical leadership to define workflows, performance measures, and service delivery strategies Supervise and mentor a Bilingual Community Health Worker, supporting outreach and behavioral health education efforts Care Coordination & Documentation Develop and maintain individualized treatment plans and clinical records Ensure timely documentation, assessments, and re-authorizations Actively participate in integrated care planning and multidisciplinary collaboration Why Work at CommuniCare?Mission-Driven Impact:
