As a MDS Case Management professional, you will play a vital role in ensuring the highest quality of care for our residents. Working closely with our healthcare team, you will be responsible for conducting comprehensive assessments, developing individualized care plans, and coordinating with healthcare professionals to achieve optimal outcomes. About the
Role:
As a MDS Case Management professional, you will be responsible for: Conducting comprehensive assessments of residents to identify their needs and develop individualized care plans Collaborating with healthcare professionals, including physicians, nurses, and therapists, to develop and implement care plans Coordinating with interdisciplinary teams to ensure that care plans are carried out effectively Documenting assessments, care plans, and progress in accordance with regulatory requirements Maintaining accurate and up-to-date records of resident care and progress
Requirements:
MDS Case Management experience preferred Bachelor's degree in a related field (n/a) Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment Familiarity with MDS guidelines and regulations
