Job Description

Required Experience/Skills Graduate of an accredited School of Nursing. Registered Nurse (RN) license in good standing, required. Clinical leadership experience in a Medicare-certified home health agency. Strong understanding of PDGM, QAPI and care coordination. Passion for delivering high-quality patient-centered care in a home setting. Must know OASIS and two years as a Registered Nurse with at least one year of management experience in home care.
