Job Description Job Description Position Summary We are seeking a detail-oriented Medical Assistant to support our participation in an Accountable Care Organization (ACO) program .This role blends traditional clinical support with population health, quality reporting, and coding-related responsibilities. The ideal candidate understands CPT and ICD-10 diagnosis codes and is comfortable working with care coordination and quality measures. Key Responsibilities Support ACO initiatives including quality measures, care gaps and population health workflows Assist with accurate documentation and review CPT and ICD-1- diagnosis codes Identify and flag care gaps (e.g., preventative screenings, chronic condition follow-ups) Coordinate patient outreach related to ACO quality measures and annual wellness visits Assist providers with clinical documentation to support accurate coding and compliance Maintain accurate patient records in the EHR Communicate with patient regarding appointments, labs, referrals, and care plans Collaborate with clinical and administrative staff to meet ACO performance goals Ensure compliance with HIPAA and regulatory requirements Qualifications Certified or registered Medical Assistant preferred Experience in working a primary care or value-based care setting Working knowledge of CPT and ICD-10 diagnosis codes Familiarity with ACO, Medicare, or value-based care programs Strong attention to detail and organizational skills Comfortable using electronic health records (EHRs) Excellent communication and teamwork skills
