Job Description:
Leverages reports and dashboards to monitor the effectiveness of the Care Management program and create strategies to improve outcomes. Manages Acute Care Management, Post-Acute Care Collaborative, and Operations Optimization across market hospital campuses. Mentors and coaches subordinates to deliver high-quality, cost-effective services. Provides leadership support for IT initiatives to develop, plan, implement, and support long-term needs. Participates in Acute Care Services, Capacity, and Medical Management committees. Maintains knowledge of federal and regulatory policies in patient care management and reimbursement. Collaborates with regulatory and public policy departments to advocate for hospital and government entities to improve public policy and requlations. Oversees compliance, reimbursement, and internal audit plans, leveraging dashboards and reports to monitor Care Management outcomes. Provides leadership and management of the development and adherence of an annual operational and capital budget for areas of responsibility. Other duties as assigned. Leads and coordinates clinical care management activities, transitional planning, and management of progression of care strategies. Sets the program's vision, mission, strategic objectives, and goals. Collaborates with clinical leaders to manage length of stay, reduce avoidable readmissions, and improve the patient experience.

Education:
  • Bachelor's [Required]

Field of Study:
  • in Nursing or Health Related Field
  • in business or healthcare related field
Work Experience:
  • 4+ of progressive leadership experience [Required]
  • 6+ functional health care setting [Required]
