Patient Care Coordinator

Kids Place PPEC is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our growing team as a Patient Care Coordinator . This role is ideal for a nurse who thrives in a fast-paced pediatric environment and enjoys collaborating with hospitals, physician offices, families, and interdisciplinary care teams to ensure exceptional patient care.

Responsibilities:

Coordinate and manage confidential patient plans of care Communicate regularly with hospitals, physician offices, specialists, and case managers Maintain accurate patient records and ensure compliance with healthcare regulations Assist with admissions, authorizations, care coordination, and medical documentation Collaborate with nursing staff and therapists to support continuity of care Ensure all patient information is handled with the highest level of confidentiality Build positive relationships with families and healthcare providers Support daily clinical operations as needed

Qualifications:

Active Florida RN License required Pediatric experience preferred Strong knowledge of medical terminology and clinical documentation Experience coordinating care plans and communicating with medical providers Excellent organizational and multitasking skills Strong interpersonal and communication abilities Ability to work independently and as part of a team Experience in PPEC, home health, pediatrics, or case management is a plus
