Job Description
Department Inpatient Rehab Facility (IRF) Job Summary The Rehab Clinical Case Manager assesses the patients' needs upon admission and assists with the coordination of interdisciplinary treatment plans based on CMG, estimated length of stay, and functional efficiency goals to ensure achievement of predicted outcomes, as well as monitoring and updating the interdisciplinary care plans as appropriate. Required Qualifications
- Require a diploma or Associate of Science in Nursing (ASN) with graduation from an accredited school of nursing. A Bachelor of Science in Nursing (BSN) is preferred.
- Require a minimum of one (1) year of Utilization Review (UR) or Case Management experience in a hospital setting and a minimum of three (3) years of related clinical practice in an acute care or inpatient rehab setting.
- Require previous experience with hospital information systems (order entry, results reporting, case management) as well as proficient experience using commonly used computer applications (word processing a minimum). Preferred Qualifications
- Prefer active Case Management Certification (CCM or ACM) or within one (1) year of hire and membership in relevant CM organization.
- Prefer demonstrated ability to manage multiple tasks and adjust priorities according to patient needs and department/hospital needs.
- Prefer solid clinical assessment, critical thinking, decision making and organizational skills.
- Prefer strong ability to communicate effectively both verbally and in written work.
- Prefer strong interpersonal skills and ability to work collaboratively with leaders, staff, patients, families, healthcare team, payers and external agencies.
