Job Description

WHO WE ARE
At Breakthru, our mission is to improve access to mental health care with transformative interventional psychiatric treatments in a human-centered environment where patients are truly seen and heard. We specialize in treatment-resistant mental health conditions, delivering innovative solutions — including Ketamine infusions, TMS, PRISM and Spravato — for patients who have not responded to standard care, in an out-patient setting. Breakthru does not offer medication management or talk therapy and collaborates with local psychiatrists and therapists.
WORK SCHEDUULE
Monday to Friday; 8 hour days, 30-minute lunch, start time between 8:15 am and 9:00 am
POSITION SUMMARY
The EMT/Clinician is a key member of the clinical team, responsible for the safe and skilled delivery of interventional psychiatric treatments. The defining differentiator of this role is the EMT's ability to independently start and manage intravenous (IV) lines — a core clinical skill that directly enables Breakthru's Ketamine infusion program and the high-quality care experience our patients depend on. In addition to IV therapy, this role supports TMS (Transcranial Magnetic Stimulation) treatments and Spravato (esketamine) administration. The EMT will work alongside psychiatrists and clinical staff in a structured, fast-paced outpatient setting.
KEY RESPONSIBILITIES 1. IV
Therapy & Infusion Management Independently start peripheral IVs on patients for Ketamine infusion treatments Set up treatment rooms with IV bags, infusion supplies, and necessary equipment daily Monitor patients throughout infusion — vitals, neurological status, and comfort — and document in Athena Respond appropriately to any adverse reactions during infusion and escalate as needed Maintain up-to-date medication logs after each patient treatment (dosing/usage) Accept and log incoming medications per protocol 2. TMS & Spravato Treatment Support Administer and monitor Transcranial Magnetic Stimulation (TMS) sessions after training Administer Spravato per REMS protocol, including required enrollment and documentation Track and reconcile TMS visits in both Athena and Magventure Ensure the TMS device is properly turned on/off and maintained daily Complete Spravato REMS and 'Spravato & Me' enrollment forms 3. Clinical Documentation & Chart Management Complete clinical journal entries and daily treatment notes in Athena Prep patient charts 48 hours in advance of scheduled treatments Complete patient intake cards daily Review office schedule daily and call new treatment patients Follow up with Spravato and Ketamine patients after treatment 4. Administrative Support Assist psychiatrist with patient communication when not engaged in treatments Any additional tasks as assigned such as drafting clinical team assignments, assisting with special projects or supporting operational initiatives. 5. Facility & Supply Shared Duties Stock treatment rooms at end of each day Ensure all rooms are clean, sanitized, and ready for next day Manage medical supply ordering Maintain and update the SDS (Safety Data Sheet) book as needed Update and maintain treatment protocols/templates Manage laundry and general clinical housekeeping tasks Turn on/off lights, coffee, and music as part of opening/closing routine
CORE COMPETENCIES
Demonstrated skill in peripheral IV placement — confident, calm, and patient-centered technique Clinical judgment and ability to recognize and respond to patient changes during treatment Compassionate, emotionally intelligent approach with treatment-resistant psychiatric patients Strong prioritization and efficiency across simultaneous patient treatments Values patient confidentiality and HIPAA compliance in all interactions Resilient and resourceful in a fast-paced, growing multi-site practice Collaborative team member who actively supports other clinical and administrative staff
EDUCATION & EXPERIENCE REQUIREMENTS AEMT/EMT
certification required — active and in good standing Current certification must be renewed before expiration date ACLS and BLS certifications required Previous infusion experience required; IV therapy proficiency is mandatory for this role 1-2 years of clinical experience in a hospital, clinic, or EMS setting preferred Experience in a psychiatric, infusion, or specialty outpatient practice is a strong plus Familiarity with EMR systems (Athena experience preferred)
