This vital role involves overseeing patient care delivery, managing Care staff, ensuring compliance with healthcare regulations, and maintaining high standards of quality and safety. Responsibilities Oversee daily operations of the home health care program, ensuring seamless delivery of patient services aligned with regulatory standards. Supervise care staff, including nurse management and case management teams, providing guidance, mentorship, and performance evaluations. Coordinate patient care plans, and document care records accurately and efficiently. Ensure compliance with HIPAA regulations and maintain confidentiality of all medical records and patient information. Manage staffing schedules, and resource allocation to optimize service delivery. Collaborate with interdisciplinary teams including hospice care providers, senior care specialists, and hospital partners as needed. Facilitate training on medical terminology, medical records documentation, and healthcare management best practices for staff development and training. Monitor quality assurance metrics related to patient care outcomes, and case management processes to uphold high standards of service. Requirements Valid Registered Nurse (RN) license with current state certification. Demonstrated supervisory experience managing nursing teams within assisted living facilities, nursing homes, or home health agencies. (recommended not required) Strong background in patient care across diverse environments including hospital medicine, hospice care, acute care, and senior living settings. Familiarity with medical terminology, HIPAA compliance, and medical records management. Experience with healthcare management tasks such as utilization review, case management, and staffing coordination. Excellent communication skills with the ability to lead teams effectively while maintaining a compassionate approach to patient-centered care.
