Job Description

Overview We are seeking an experienced and dedicated LPN or RN for our Director of Health and Wellness to lead our healthcare team in providing exceptional patient care within a dynamic healthcare environment. The ideal candidate will possess extensive clinical expertise, leadership skills, and a comprehensive understanding of healthcare management, including compliance with HIPAA and Medicare regulations. This leadership role involves overseeing care delivery, managing staff, ensuring documentation accuracy, and fostering a culture of excellence in resident-centered care. The Director of Health and Wellness will play a pivotal role in developing care plans, supervising nursing staff, and maintaining high standards across all health services.
BASIC FUNCTION
:

The Wellness Director is responsible for coordinating resident care in order to ensure the residents remain as independent as possible for as long as possible in the Traditional Assisted Living and Secure Memory Care sections of the community. Assist the resident and his or her family in maintaining the physical and emotional health of the residents. Liaison with hospital personnel, physicians, community organizations and other health related service agencies to provide care to the residents. Hire, evaluate, coordinate, motivate, monitor performance, schedule and supervise department staff in accordance with company policy. Make recommendations to the Executive Director with respect to staff evaluations, merit pay increases, reprimands, disciplinary actions and terminations. Responsibilities Lead and supervise nursing staff, including nurse management and case management teams, ensuring quality care delivery aligned with resident needs. Develop, implement, and review individualized care plans for residents, incorporating physiology and anatomy knowledge to optimize health outcomes. Oversee patient monitoring activities, ensuring timely intervention and appropriate documentation review within EMR systems. Manage shift scheduling and shift management to ensure adequate staffing levels for all operational hours. Ensure compliance with HIPAA regulations and maintain the confidentiality of medical records and documentation. Coordinate with interdisciplinary teams to deliver comprehensive care in assisted living, home health, hospice care, acute care, and home care settings. Oversee budgeting processes related to health services operations while maintaining cost-effective practices without compromising quality. Monitor regulatory compliance related to Medicare and other healthcare standards to ensure ongoing accreditation. Provide leadership in training staff on medical terminology, documentation procedures, and resident safety protocols. Foster a culture of continuous improvement by reviewing clinical documentation and implementing best practices for resident care. Requirements Proven management experience with supervising healthcare teams in assisted living, home health, hospice care, or acute care settings. Critical care experience preferred; familiarity with home care operations is a plus. Strong knowledge of physiology, anatomy, nursing practices, medical terminology, and case management principles. Experience with EMR systems and medical records management is essential. Demonstrated leadership skills with the ability to motivate staff and oversee shift management effectively. Knowledge of HIPAA regulations, Medicare policies, and healthcare compliance standards. Excellent organizational skills with experience in budgeting and resource allocation within healthcare settings. Valid nursing license; nurse management experience highly desirable. Ability to review clinical documentation accurately while maintaining attention to detail in patient monitoring and care planning processes.
