02/22 I.JOB SUMMARY/RESPONSIBILITIES
Plans, coordinates and screens for clinical appropriateness for inpatient transfers. Responsible for transfer process and adherence to requirements including State and Federal regulations. Collaborates with physicians, hospital staff, insurance companies, and patient/family. Coordinates and acts as liaison to facilitate and manage appropriate and timely health care services. Coordinates acceptance or denial of interfacility transfers with physicians based on clinical need and Medical Center resources. In collaboration with Bed Control, reviews direct and emergency admissions for clinical and financial appropriateness and assists with bed coordination and census management. Performs all admission screenings and documentation for clinical appropriateness using evidence based guidelines.II. TYPICAL PHYSICAL DEMANDS

Not substantially subjected to adverse environmental conditions. Some exposure to communicable diseases. Work pace varies and is frequently stressful due to census, patient account types and time frames.IV. MINIMUM QUALIFICATIONS
A.EDUCATION/CERTIFICATION AND LICENSURE
Current Hawaii State License as a Registered Nurse. Knowledge of Emergency Medical Treatment Active Labor Act (EMTALA) preferred.B. EXPERIENCE
