Home - Fortis Job Summary We are seeking an Assistant Clinical Manager to support the clinical operations of Select Home Health.
This position serves as a key resource to the Director of Nursing by assisting with electronic medical record (EMR) workflows, clinical documentation review, quality assurance and performance improvement initiatives, regulatory compliance activities, and various clinical and administrative functions. The Assistant Clinical Manager collaborates with the interdisciplinary team to promote high-quality patient care, ensure compliance with agency policies and regulatory standards, and support the overall efficiency and effectiveness of clinical operations.
Duties/Responsibilities:
Clinical Operations Support Assist with Homecare Homebase (HCHB) clinical workflows and daily operational processes. Support clinicians with documentation requirements, workflow navigation, and clinical system utilization. Monitor workflow queues and communicate issues requiring leadership intervention. Assist with implementation of process improvements related to clinical operations. Manage tracking systems that are external from HCHB regarding clinical operations. Perform other duties as assigned to support agency operations and patient care excellence. Quality Assurance & Chart Auditing Perform routine chart audits to ensure compliance with agency, state, federal, and payer requirements. Identify trends and opportunities for clinician education and process improvement. Communicate audit findings and assist with corrective action plans as directed. Reporting & Data Management Generate, distribute, and monitor clinical performance reports. Assist leadership with data collection and analysis related to quality indicators, productivity, compliance, and outcomes. Maintain accurate records and tracking tools to support agency initiatives. Staff Development & Education Assist in the development and delivery of staff education and training programs. Participate in orientation and onboarding activities as assigned. Quality Improvement & Compliance Serve as an active participant in the agency's Quality Assurance Performance Improvement (QAPI) program. Participate in Compliance Committee activities and initiatives. Support implementation and monitoring of corrective action plans and performance improvement projects. Physical Requirements Prolonged periods of sitting at a desk and working on a computer/tablet Must be able to lift 15 pounds at times. Regularly required to sit, talk, hear, and use fingers to operate a computer and telephone
