Key responsibilities include:
Clinical Oversight Ensure standardized triage protocols are followed across all Access Center programs Provide clinical escalation support and decision‑making guidance Lead quality reviews, incident follow‑up, and documentation compliance Leadership and Team Development Directly supervise two Team Leads, including performance management and competency oversight Operational leadership for Deaconess Care Transition Team RNs Support onboarding, education, and ongoing clinical development of nursing staff Promote accountability, engagement, and retention within the Access Center nursing teams Operational Efficiency & Process Improvement Identify workflow gaps and redundancy across programs Lead initiatives to streamline call handling, triage pathways, and care transitions Collaborate with Ambulatory leadership, Post Acute leadership, and Hospital teams to improve handoffs and throughput Support organizational goals related to quality, patient experience, and care transitions Reporting and Performance Monitoring Develop and deliver monthly reporting on volume, response time, outcomes, and quality indicators Monitor KPIs related to triage accuracy, patient safety, escalation appropriateness, and staff productivity Use data to drive continuous improvement and inform leadership decisions Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
