Fort Wayne Office Position Details:

Will work out of Fort Wayne office daily.

M-F 8A-5P.

Will participate in back-up rotation for afterhours MOD and Holidays & PTO when needed.

Job Responsibilities:

Manage Hospice Medical Records blast for your team and follow up on all requests in a timely manner including providing feedback to team member when requests are completed. Complete all routine plotting duties and incoming plotting requests as needed and in a timely manner. Manage all DME orders, pick-ups and follow-up requests as needed in a timely manner. Call funeral homes daily on any previous day patient deaths and follow death certificate physician verification process. Order x-rays & labs as requested. Answer large amounts of incoming calls from patients, families and referral sources and show professionalism and competency in assisting and/or directing calls to the appropriate department.

Experience:

One year of Hospice experience preferred, but not necessary CNA License is preferred, but not necessary Experience in medical records and working in an office setting Experience in customer service

Credentials:

Preferred Licensed Certified Nursing Assistant in State of Indiana Current driver's license Must have reliable transportation and insurance
