The Quality and Patient Safety Coordinator supports system-wide implementation, coordination, evaluation, and sustainment of Beacon Health System's High Reliability Organization (HRO) framework, patient safety initiatives, quality improvement programs, and risk mitigation strategies. The role collaborates with Patient Safety, Quality, Risk, Performance Improvement, clinical leaders, operational teams, and medical staff to advance a culture of safety, reliability, and continuous improvement. It applies standardized tools to assess systems, reduce variation, anticipate and mitigate risks, and drive measurable gains in patient outcomes, safety, and operational excellence. Provides Quality, Patient Safety, and High Reliability Program Support by Leading system-wide implementation, coordination, and maintenance of Beacon Health System's High Reliability framework and patient safety programs. Collaborating with Quality, Patient Safety, Risk, and Performance Improvement teams to embed a culture of safety and reliability organization-wide. Monitoring patient safety event reporting systems to identify trends, near misses, and improvement opportunities. Coordinating system-level safety reviews, including root cause analyses (RCAs), learning reviews, and investigations under Patient Safety leadership. Conducting proactive risk assessments such as Failure Mode and Effects Analyses (FMEAs) to identify and mitigate vulnerabilities. Developing, implementing, and tracking corrective and preventive action plans to address safety and reliability risks. Participating in safety huddles, High Reliability rounds, and interdisciplinary forums to promote situational awareness and speaking-up behaviors. Fostering a just and psychologically safe culture focused on learning from errors and near misses. Coordinating meetings, committees, and workgroups related to quality, patient safety, and High Reliability, including agenda preparation and documentation. Supporting accreditation readiness, regulatory compliance, and survey preparation for patient safety and quality standards. Leading the design, delivery, coordination, and evaluation of High Reliability education and training programs across Beacon Health System. Creating and facilitating educational sessions on Beacon-identified HRO principles for all staff levels, including clinical, non-clinical, leaders, and medical staff. Serving as the system-wide resource for High Reliability education, aligning content with organizational priorities, regulatory requirements, and evidence-based practices. Partnering with Physician Leadership, Medical Staff Services, Education, and operational leaders to drive medical staff engagement in quality and safety initiatives. Data Management, Analysis, and Reporting Collecting, tracking, validating, trending, and reporting High Reliability, patient safety, and quality metrics. Analyzing safety and reliability data to detect patterns, emerging risks, and improvement opportunities. Preparing dashboards, reports, and presentations that translate data into actionable insights for frontline teams and leadership. Promoting transparency and timely communication of safety and quality performance across the organization. Responding promptly to data requests from physicians, leadership, and staff. Conducting internal and external benchmarking to adopt best practices. Education, Engagement, and Performance Improvement Planning, coordinating, and facilitating ongoing High Reliability education sessions, workshops, and learning forums.Reinforcing the five HRO principles:
preoccupation with failure, sensitivity to operations, reluctance to simplify, commitment to resilience, and deference to expertise. Supporting onboarding and refresher training in High Reliability for all staff and medical staff. Aligning High Reliability initiatives with the Beacon Operating System and other performance improvement efforts. Facilitating spread of best practices and sustained improvements across departments and sites. Performs other functions to maintain personal competence and contribute to departmental effectiveness by completing assigned projects and duties as directed.ORGANIZATIONAL RESPONSIBILITIES
Attendance at department meetings and accountability for shared information. Timely completion of mandatory education, annual competencies, and department-specific training. Annual employee health requirements. Maintenance of any required license/certification in good standing. Adherence to regulatory, survey, and compliance standards. Compliance with all organizational and department policies. Availability for overtime or adjusted schedules as needed. Education and Experience Bachelor's degree in healthcare administration, nursing, public health, quality, business, or related field required. 1 - 3 years of experience in a Performance Improvement role is required. One to three years of experience in patient safety, quality, performance improvement, program coordination, or outcomes/data management preferred. Knowledge & Skills Working knowledge of High Reliability and patient safety principles. Strong analytical, problem-solving, and critical thinking abilities. Excellent organizational skills with the ability to manage multiple priorities and deadlines. Effective verbal, written, listening, and interpersonal communication skills. Proven ability to collaborate across disciplines and organizational levels. Proficiency in Microsoft Office (Word, Excel, PowerPoint, Copilot) and data reporting tools. Ability to handle confidential and sensitive information with discretion. Working Conditions Primarily office-based with periodic clinical setting interaction.
