Job Description

Opportunity Summary As our Medical Case Coordinator , you will coordinate and monitor low-acuity client cases to ensure timely, accurate, and client-centered care, promote healthcare compliance, and improve outcomes for vulnerable families and children. Key Responsibilities Every day, you will manage referrals and referral tracking processes, maintain and update referral spreadsheets and CMP records, and support care coordination activities across internal and external providers. You will attend client appointments and low-acuity visits as needed, assist with follow-up care, and ensure documentation is completed accurately and in compliance with organizational standards. Must be comfortable with home visits and transporting clients in vehicle. Success Factors To be successful in this role, you must be clinically sharp and relationally warm, flexible and adaptable, highly organized with close attention to detail, and passionate about empowering families. Additionally, this role requires effective communication and the ability to manage multiple tasks while supporting clients through the care coordination process. Qualifications Certified Medical Assistant (AAMA preferred) Associate degree preferred Minimum 3-5 years of patient care experience as a CMA Experience with abulatory care CPR/First Aid certification Valid driver's license and auto insurance Strong communication and time-management skills Strong computer skills Who We Are Hardon Education is an accredited career college providing healthcare education, school nursing services, and nursing case management to support community health. Pay:
$27.00 - $30.00 per hour
