Duties include teaching activities of daily living skills and linking clients with community resources. The Care Manager will provide services to adults and kids/adolescents with mental illness and/or addiction through individual and group modalities. A Bachelor's degree in social work, psychology or related field is desired. Experience will be considered in lieu of education. We are looking for caring, compassionate and energetic people who enjoy helping others. An electronic medical record is used so computer literacy is also required.
