Hospice Patient Care Manager RN -Valparaiso, Indiana Full-time with supervisor on-call rotation Make a meaningful impact. Lead with compassion. Grow with purpose. Our hospice organization is seeking a dedicated Patient Care Manager (PCM) RN to guide our interdisciplinary team and ensure exceptional care for every patient we serve. What You'll Do Lead and mentor a team of nurses, aides, social workers, and chaplains Oversee patient care plans and ensure regulatory compliance Support families with empathy, clarity, and professionalism Collaborate closely with physicians and clinical leadership Foster a culture of compassion, accountability, and excellence
Requirements:
