Job Summary We are seeking a dedicated and experienced Hospital Case Manager to coordinate patient care within our facility. The ideal candidate will possess a strong background in acute and post-acute care, with expertise in managing complex cases across diverse medical settings. As a Case Manager, you will facilitate patient assessments, discharge planning, and utilization management, ensuring seamless transitions of care while maintaining compliance with healthcare regulations and promoting optimal patient outcomes. Duties Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations, to determine individual care needs. Collaborate with multidisciplinary teams—including nursing, physicians, social workers, and therapists—to develop personalized care plans aligned with patient goals. Coordinate inpatient and outpatient services, ensuring timely diagnostic evaluations and appropriate utilization of hospital resources. Manage discharge planning processes by evaluating post-acute care options such as skilled nursing facilities or outpatient clinics to support safe transitions. Monitor hospital records and medical documentation for accuracy and compliance with HIPAA regulations while maintaining detailed case notes. Facilitate communication between patients, families, and healthcare providers to promote understanding of treatment plans and care objectives. Qualifications A Masters degree in Social work, Psychology, or other Mental Health field is preferred. A BA degree in Mental Health or Human Services related field or a Bachelor's degree with 2 years experience in a Psych Hospital or a degree in Nursing and licensure by Kentucky as a Registered Nurse or Licensed Practical Nurse is required. Proven experience in hospital case management within a Level I or Level II trauma center environment; ICU or emergency room experience is highly desirable.
