Job Description

  • Role Overview:
  • The Supervisor, Integrated Case Management, oversees the daily operations of the Case Management unit, providing clinical direction, guidance, and oversight of case management services.
  • Work Arrangement:
  • + Remote - Associate must reside in Louisiana (LA)
  • Responsibilities:
  • + Provide daily oversight and clinical direction for the Complex Case Management unit.
+ Supervise and support staff conducting case management, care coordination, and screening activities. + Monitor team performance, productivity, and quality to ensure compliance with organizational and regulatory standards + Manage and track case management programs + Complete internal staff audits + Monitor contract compliance + Assist in the implementation and execution of departmental goals, workflows, and operational plans + Serve as a resource to staff for clinical and operational guidance + Ensure timely and accurate documentation, reporting, and case management activities + Collaborate with internal teams and leadership to support member care and service delivery + Identify opportunities for process improvement and support workflow efficiencies + Participate in staff training, coaching, and performance management activities + Support occasional field work alongside care managers and community-based staff + Management and oversight of specialist projects as required.
  • Education & Experience:
  • + Bachelor of Science in Nursing is required + Unrestricted licensed Registered Nurse (RN) in LA + 1 to 3 years of supervisory or field leadership experience + 3 to 5 years of case management experience supporting care management in a clinical or managed care environment + Designation of Certified Case Manager is preferred.

  • + Unrestricted licensed RN in LA license
  • Skills & Abilities:
  • + Strong clinical knowledge of case management and care coordination practices + Leadership skills with the ability to coach, mentor, and develop staff + Ability to manage multiple priorities in a fast-paced environment + Strong organizational and problem-solving skills + Knowledge of healthcare regulations, standards, and compliance requirements + Effective communication and interpersonal skills across cross-functional teams + Ability to analyze workflows and implement process improvements + Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, and Teams)As a company, we support internal diversity through: Recruiting.
