The Director of Clinical Services is responsible for the overall leadership, coordination, and management of clinical operations within the PACE program. This position oversees the delivery of high-quality participant care, ensures compliance with CMS, LDH, Medicare, Medicaid, and PACE regulations, and promotes an interdisciplinary team approach to participant-centered care. The Director of Clinical Services works closely with providers, nursing staff, therapists, social services, and operational leadership to ensure safe, effective, and compassionate care for frail elderly participants while supporting PACE's mission of helping participants remain safely in the community. Essential Duties and Responsibilities Oversees all clinical operations and ensures quality participant care in accordance with PACE regulations and organizational policies. Supervises and provides leadership to nursing staff, clinic staff, therapists, and other clinical team members. Collaborates with the Interdisciplinary Team (IDT) to develop, implement, and monitor individualized participant care plans. Ensures compliance with CMS, LDH, Medicare, Medicaid, OSHA, infection control, and all regulatory requirements. Monitors clinical outcomes, participant satisfaction, incident reports, hospitalizations, grievances, and quality improvement initiatives. Assists with preparation for CMS, LDH, and regulatory audits and participates in corrective action planning when necessary. Oversees clinical documentation to ensure accuracy, timeliness, and compliance. Coordinates participant care services including clinic services, home care, specialty care, therapy services, and nursing support. Provides clinical guidance and support to staff regarding participant care issues and best practices. Participates in hiring, orientation, training, coaching, evaluations, and disciplinary actions for clinical staff. Ensures proper staffing levels to meet participant care needs and operational requirements. Promotes participant safety and risk management initiatives. Assists with development and implementation of clinical policies, procedures, and workflows. Collaborates with the Medical Director and senior leadership regarding participant care, staffing, and clinical operations. Maintains knowledge of current healthcare regulations, geriatric care standards, and PACE requirements. Participates in meetings, committees, and interdisciplinary care conferences as required. Education Registered Nurse (RN) required; BSN required, MSN preferred. Current unrestricted Louisiana RN license required. Experience Minimum of five (5) years of clinical leadership or management experience. Minimum of three (3) years of experience working with geriatric, long-term care, home health, PACE, hospice, or managed care populations preferred. Experience with interdisciplinary team coordination strongly preferred. Experience with CMS and regulatory compliance preferred. Knowledge, Skills, and Abilities Strong knowledge of geriatric care and chronic disease management. Knowledge of PACE regulations, Medicare, Medicaid, and long-term care services. Excellent leadership, organizational, and communication skills. Ability to manage multiple priorities in a fast-paced healthcare environment. Strong problem-solving and decision-making abilities. Ability to maintain confidentiality and professionalism. Proficient in electronic medical records (EMR) and Microsoft Office applications. Physical Requirements Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed. Ability to travel between participant homes, clinic sites, hospitals, and nursing facilities as necessary. Working Conditions Clinical and office environment with occasional participant home visits and community travel. May require occasional evening, weekend, or on-call responsibilities based on operational needs. Reporting Structure Reports to:
Executive Director and/or Medical Director Supervisory Responsibilities Directly supervises clinical staff including nursing personnel, clinic staff, and other assigned clinical team members.
