Qualifications Delegation Full Job Description Job Responsibilities:
1. Manages a system for coordinating the care of cancer center patients throughout the entire course of treatment and including post-treatment services as needed to ensure optimal care for the patient. 2. Coordinates referrals with physicians, other ancillary departments and outside agencies. 3. Maintains an appropriate communication system through documentation and interaction with the patient, physicians, family, and other members of the health care delivery team. 4. Demonstrates leadership skills in effective planning, time management, delegation, prioritizing and feedback. 5. Contributes to a positive work environment and to the overall team effort of efficient care delivery. 6. Demonstrates an ability to plan and apply the care management process for patients with "age-related" problems to include pediatric and geriatric assessment in accordance with standards of care and other health system department's departments policies and procedures. 7. Works collaboratively with all staff members, communicate, encourage, and communicate with all staff members involved in patient care delivery activities to identify opportunities for improvement. 8. Understands each discipline's needs and objectives and coordinates these with the needs of the patient to develop an effective plan of care. Implement departmental referrals when necessary, ie., dietitian, physical therapy, community-based referrals. 9. Documents case management assessments, interventions and outcomes identified on progress and/or patient notes; notes to be reviewed by interdisciplinary team members. 10. Provides ongoing education and or technical information and assistance to medical staff, physician office staff and various health system personnel, when necessary. 11. Maintains and promotes a positive attitude even in adverse or difficult situations. Handles conflict in a professional manner.
Education Requirements:
LCSW, MSW, LPC, or RN
