Vizx Corp is conducting a confidential search for an Executive Director to lead an established, CARF-accredited MassHealth provider of Adult Foster Care (AFC) and community-based home services in Greater Boston. This is a true executive appointment — full P&L accountability, regulatory authority, and strategic leadership of a multi-disciplinary organization at a pivotal moment in its growth. The Executive Director will serve as the senior-most on-site leader, reporting directly to the Vizx Corp CEO. You will inherit an organization with a strong clinical reputation, trusted caregiver relationships, and a deep community presence — and you will be expected to build on that foundation with the operational discipline and strategic vision needed to scale. This role is for an executive who has run a healthcare organization before — not someone aspiring to. If you have led an AFC provider, a home health agency, an FQHC, or a similarly complex MassHealth-contracted program and are ready to step into full organizational accountability, we want to speak with you. SCOPE OF LEADERSHIP
This is a full organizational leadership role. The Executive Director oversees:
1. Clinical & Program Operations AFC and GAFC program delivery across all enrolled members MDT structure: Program Director, RN/LPN, and Care Manager functions Caregiver recruitment, evaluation, training, and ongoing support Clinical assessments, prior authorizations, and care plan workflows Financial & Administrative Oversight Full P&L accountability for the organization MassHealth revenue cycle: billing, claims, AR aging, prior authorization Budget management and vendor relationships Regular financial reporting to Vizx Corp leadership 2. Regulatory & Compliance Authority MassHealth AFC/GAFC
compliance (130 CMR 408.000) and billing standards CARF accreditation ownership — readiness, surveys, corrective action HIPAA compliance and data governance Incident reporting and quality management under EOHHS requirements Workforce & Organizational Leadership Executive leadership across clinical, administrative, and care management teams Talent strategy: hiring, retention, succession planning Culture ownership — accountability, trust, and staff engagement Organizational transition leadership in partnership with Vizx Corp 3. Clinical Operations & Service Delivery Lead delivery of services across all program lines with full accountability for member outcomes, caregiver quality, and program integrity Maintain the multidisciplinary team (MDT) structure required under applicable regulations — ensuring each function is appropriately staffed, supervised, and performing Oversee caregiver and direct care worker recruitment, onboarding, annual evaluation, and ongoing support to sustain program quality and member safety Ensure timely completion of all clinical documentation: assessments, prior authorizations, care plans, and discharge planning Monitor clinical quality indicators and drive continuous improvement across all care delivery programs 2. Regulatory Compliance & Risk Management Maintain full compliance with MassHealth AFC/GAFC
regulations (130 CMR 408.000), all billing rules, and administrative standards Own CARF accreditation — including policies, internal monitoring, mock survey preparation, and corrective action planning Oversee HIPAA compliance, mandated reporting obligations (51A), and incident management Monitor EOHHS and MassHealth regulatory developments and implement policy adjustments proactively Serve as the organization's primary point of contact with EOHHS, MassHealth, and CARF surveyors 3. Financial Leadership Own the organization's P&L — managing budget, cost structure, and financial performance to margin and growth targets set with Vizx Corp leadership Oversee revenue cycle performance: billing accuracy, claims submissions, AR aging, prior authorization compliance, and payer reconciliation Identify and execute opportunities for financial efficiency, service-line growth, and sustainable scaling Deliver regular, board-quality financial performance reporting to Vizx Corp leadership 4. Workforce Leadership & Organizational Culture Lead, develop, and retain a high-performing executive and clinical team — with clear expectations, ongoing feedback, and visible accountability Own talent strategy: workforce planning, succession readiness, and key-person risk mitigation in a competitive labor market Build and maintain a culture of mutual respect, clinical excellence, and organizational pride across a diverse workforce Serve as a stabilizing, accessible executive presence for staff, caregivers, and community partners throughout organizational transition 4. Stakeholder & Community Relations Maintain and grow referral relationships with hospitals, discharge planners, PCPs, social workers, and community-based organizations across Greater Boston Serve as primary executive representative with MassHealth, MCOs, ICOs (Integrated Care Organizations), and SCOs (Senior Care Options) Build relationships with PACE programs and LTSS coordination entities serving Vizx Corp members Communicate proactively with members, caregivers, and families in their primary language where possible 5. Organizational Transition & Integration Partner with Vizx Corp's Director of Administration & Operational Systems on system onboarding, policy harmonization, and operational alignment Build the executive team and operational infrastructure required for sustained performance at scale in the Boston, MA market Develop and execute a 12-month strategic roadmap anchored in quality outcomes, regulatory compliance, and program growth QUALIFICATIONS & LICENSURE
