Care Manager HomeWell Care Services Baltimore, MD Job Details Full-time $23 - $25 an hour 8 hours ago Benefits Mileage reimbursement Professional development assistance Qualifications Nursing High school diploma or GED Organizational skills Associate's degree Associate Degree in Nursing Full Job Description Position Summary A Care Manager coordinates and oversees services for clients to ensure their physical, emotional, and social needs are met. They assess client needs, develop care plans, coordinate services, monitor progress, and serve as a liaison between clients, families, caregivers, healthcare providers, and community resources.
Key Responsibilities:
⁠ ⁠Conduct client assessments and identify care needs. ⁠ ⁠Develop and implement individualized care plans. ⁠ ⁠Coordinate services with healthcare providers, caregivers, and community agencies. ⁠ ⁠Monitor client progress and update care plans as needed. ⁠ ⁠Maintain accurate and confidential client records. ⁠ ⁠Advocate for clients and assist with accessing available resources. ⁠ ⁠Communicate regularly with clients, families, and care teams. ⁠ ⁠Ensure compliance with company policies and applicable regulations. ⁠ ⁠Address client concerns and resolve service-related issues. ⁠ ⁠Participate in care conferences and case reviews.
Qualifications:
⁠ ⁠High school diploma required; associate's or bachelor's degree in healthcare, social work, nursing, or a related field preferred. ⁠ ⁠Previous experience in case management, healthcare, home care, or social services preferred. ⁠ ⁠Strong communication, organization, and problem-solving skills. ⁠ ⁠Ability to work independently and manage multiple cases. ⁠ ⁠Knowledge of community resources and healthcare systems. ⁠ ⁠Proficiency with electronic records and Microsoft Office applications.
Physical Requirements:
⁠ ⁠ Ability to travel to client homes and community locations as needed. ⁠ ⁠Ability to sit, stand, and walk for extended periods. ⁠ ⁠Ability to lift up to 25 pounds occasionally.
Pay:
$23.00 - $25.00 per hour
