Job Description

Clinical Performance Coordinator Summary Join our growing healthcare system at LifeBridge Health! The Clinical Performance Coordinator plays a critical role in supporting primary care teams by optimizing patient panel management, improving patient engagement, and ensuring high-quality, value-based care delivery. This position focuses on outreach to unaligned patients, coordination of transitions of care, closure of quality care gaps, and driving completion of Annual Wellness Visits (AWVs). The specialist partners closely with providers, care managers, and administrative staff to improve outcomes, patient experience, and reimbursement performance.
PANEL MANAGEMENT AND PATIENT ENGAGEMENT
•Identify and outreach to unaligned or attributed patients to establish primary care relationships.
TRANSITIONS OF CARE
•Monitor daily admission, discharge, and transfer (ADT) reports.
QUALITY GAP CLOSURE
•Identify care gaps using payer reports, registries, and EHR tools. Partner with clinical staff to ensure completion of required screenings, labs, and follow-ups.
ANNUAL WELLNESS VISITS
(AWVs)•Proactively identify eligible Medicare patients for AWVs.
BILLING AND DOCUMENTATION SUPPORT
•Ensure accurate documentation supports valuebased billing opportunities
DATA TRACKING AND REPORTING
•Maintain dashboards and reports on panel performance, TOC visits, AWVs, and care gap closure.
CARE MANAGEMENT COLLABORATION
•Be the central "hub" for the providers for care coordination concerns.
Experience Required:
2-4 years of relevant experience Experience in healthcare operations, population health, care coordination, or medical office workflows. Experience working in primary care or value-based care environments strongly preferred. Strong understanding of population health and value-based care models.

Associate's Degree Licenses And Certification :
Preferred:
Basic Life Support Certified Medical Assistant (AAMA)
