INPATIENT MATERNAL CHILD CASE

Role:
A Licensed Social Worker (LMSW or LCSW) with experience in a hospital setting, preferably supporting maternal-child health, pediatrics, women's health, and families. The role focuses on psychosocial assessment, care coordination, discharge planning, patient advocacy, health education, and interdisciplinary collaboration.Key Responsibilities:
- Conduct comprehensive psychosocial assessments of maternal, pediatric, and family populations to identify barriers to care, provide crisis intervention, and connect patients with appropriate community and healthcare resources.
- Coordinate patient care across the continuum by collaborating with physicians, nurses, and interdisciplinary team members to develop, implement, and evaluate individualized care and discharge plans.
- Educate and support patients and families on prenatal care, infant care, safe sleep practices, postpartum mental health, and available support services while advocating for resources that promote positive health outcomes.
