Job Description

The main purpose of this full-time contractual position will be to provide stabilizing services via outreach, relationship building and case management to individuals residing in Dorchester County, who are recently released from incarceration and/or currently on parole or probation. This CSPHS I position will assist program participants in identifying needs, barriers and goals through an intake assessment process and development of a support plan. Additionally, this position will assist program participants in meeting goals and objectives through skill building, program resources, and connection to community resources and services to decrease rates of recidivism. The selected candidate must possess strong written, organizational, communication and interpersonal skills.
