$40 - $50 per hour (Commensurate with experience plus benefits / on-call pay)
Job Summary:
The Clinical Manager directs the daily clinical operations of the agency, overseeing a team of field nurses and support staff. This leader ensures adherence to Medicare/Medicaid regulations, manages the patient admission process, and ensures individualized Plans of Care (POC) lead to optimal patient outcomes. We are seeking a compassionate, highly skilled, and experienced RN Clinical Manager to join our elite care team. This role is designed for a dedicated professional who thrives in a collaborative leadership environment and enjoys mentoring and supporting their team of clinicians and the community we serve.
Responsibilities Clinical Leadership & Team Management Staff Supervision:
Supervise, mentor, and evaluate field clinicians, including RNs, LPNs, and Home Health Aides.
Scheduling & Assignments:
Ensure appropriate clinician-to-patient matching and caseload distribution.
Performance Evaluations:
Conduct in-home supervisory visits and complete timely annual performance appraisals.
Direct Care:
Step in to provide direct patient care only during emergencies or exceptional operational shortages.
Patient Care & Case Management Admissions Oversight:
Review case referrals to determine patient eligibility and appropriateness for skilled home care. Plan of Care (POC): Assist clinical staff in setting measurable therapeutic goals and developing accurate POCs.
Care Coordination:
Act as the primary liaison between patients, their families, attending physicians, and the interdisciplinary care team.
Compliance, QA & Regulations Documentation Review:
Review OASIS assessments, start-of-care, and recertifications for clinical accuracy and billing compliance.
Performance Improvement:
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives, data tracking, and utilization reviews.
Qualifications Education:
Graduate of an accredited Registered Nursing program; BSN or MSN preferred.
Licensure:
Unencumbered, active RN license in the
State of Maryland Experience:
Minimum of 2-3 years of clinical nursing experience, including at least 1-2 years of experience working as a skilled RN Case Manager in Acute Home Care and at least 1 - 2 years of experience in a leadership role.
Knowledge Base:
Strong working knowledge of Home Health regulations, reimbursement systems, and OASIS documentation.
Skills:
Proficient in electronic medical records (EMR) systems, strong problem-solving skills, and excellent communication abilities.
Requirements:
