PATIENT CARE COORDINATOR

to join our field team! The PCC provides clinical and administrative supervision to the Home Health location in addition to functioning as a Home Health Registered Nurse.

The Home Health PCC is responsible and accountable for the delivery of safe and high-quality home health services and is an advocate for the patients under their care.

HOURS:

Full Time; 1.0 FTE or 80 hours per pay period. Day Shift. Experience/Qualifications Knowledge of patient care standards and practices normally acquired through completion of a Bachelor's Degree or Associate Degree from an accredited school of nursing. Possesses an active Michigan RN nursing license. Current BLS certification or obtained within 90 days of hire. Valid driver's license required. Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability. 1 or more years' experience in home health care preferred Supervisory or management experience desired.
