Role Overview We are seeking a dedicated Patient Care Coordinator to join our operations team in a facility-based role that includes regular travel. This position supports our remote patient monitoring program by coordinating and executing the recovery of medical monitoring equipment (e.g., blood pressure cuffs, glucose monitors, pulse oximeters) from patients who have completed their care plans. You'll serve as a key liaison between patients and operations, ensuring timely pickups, accurate documentation, and safe handling of equipment.
Key Responsibilities:
- Travel to patient homes and healthcare facilities across designated states to retrieve remote monitoring devices.

- Schedule and confirm pickup appointments with patients, caregivers, and clinical teams.
- Safely handle, sanitize, and return medical equipment to inventory hubs.
- Maintain detailed records of recovered devices and update tracking systems.
- Provide basic device support or education prior to pickup if needed.
- Collaborate with logistics and supply chain teams to optimize travel routes.
- Adhere to HIPAA and infection control standards during all interactions.
- Report equipment issues, losses, or patient concerns to the Director of Operations.
- When the coordinator is not traveling, retrieving and recovering devices, they will be responsible for handling calls, managing EMR uploads, responding to voicemails and device management.
Travel Scope:
- Approximately 50% travel across multiple states.
- Overnight travel may be required depending on route and patient volume.
- Travel schedule coordinated with the Director of Operations and planned in advance. Qualifications
- 2+ years in healthcare coordination, home health, or medical device support.
- Strong communication and organizational skills.
- Comfortable working independently and managing a dynamic travel schedule.
- Valid driver's license and clean driving record.
- Familiarity with EMR systems and inventory tracking tools preferred.
- Ability to lift and transport equipment (up to 25 lbs) if required.
Work Location:
In person. The role will report to the Royal Oak office when not traveling. This is an onsite role.
