Geriatrics Full Job Description Job Purpose:
The Director of Professional Services at EHM Senior Solutions is responsible for leading and managing the professional services team to ensure the delivery of high-quality services to clients. This role involves strategic planning, client engagement, and the continuous improvement of service offerings to meet the evolving needs of the senior care industry.
Key Responsibilities:
Lead and inspire a multidisciplinary home health team to deliver compassionate, compliant, and patient-centered care. Serve as the Clinical Manager under the Medicare Conditions of Participation, ensuring excellence in outcomes and documentation. Oversee quality, performance, and patient satisfaction using WellSky and CarePort systems. Partner with the Executive Director to drive operational efficiency, strategic growth, and CHAP compliance. Hire, mentor, and develop clinical supervisors and staff; ensure ongoing education and competency validation. Monitor key operational indicators to anticipate staffing needs and ensure seamless continuity of care. Lead onboarding and orientation for new clinical hires; foster a culture of learning and accountability.
Required Education:
Associate's or Bachelor's degree in Nursing, Business Administration, Healthcare Management, or a related field. A Master's degree is preferred.
Required Experience:
Current Active Michigan License as a Registered Nurse Minimum 2 years of clinical leadership or supervisory experience in a Skilled Home Health Care setting. 3 years of experience as a Skilled home care clinician or hospice clinician Proficiency in WellSky, Microsoft Office, and familiarity with CarePort or Epic preferred. A collaborative spirit and passion for leading teams that deliver exceptional care to older adults.
