Clinical Coordinator

The Care Coordinator is a RN who coordinates and directs the nursing activities of a designated unit at PioneerCare and works with adult and geriatric residents. The RN Care Coordinator is responsible for the professional assessment and coordination of care needed for the residents of a neighborhood, nursing staff and overall operation of the neighborhood to ensure a high standard of quality care. Coordinates care provided by other members of the health care team, makes referrals for follow-up care and consults with physicians and others to provide individualized care. The RN Care Coordinator supports and assures the policies and procedures of PioneerCare are adhered to and is responsible and reportable to the Director of Nursing in coordinating the total care of residents. This is a Full Time position - 40 hours a week, Monday-Friday 8 a.m. to 4:30 p.m. There is also an every 8th weekend On Call rotation. Pioneer Care is an Equal Opportunity Employer and Provider.
