Seeking an experienced, highly skilled Clinical Supervisor to lead and elevate clinical practice across our growing home health care agency. This role is designed for a clinician who combines strong leadership instincts with deep clinical expertise and a are committed to providing high-quality, patient-centered care. The Clinical Supervisor will oversee a multidisciplinary team, ensure regulatory compliance, and drive continuous improvement in patient outcomes and agency quality initiatives. We are looking for strong Therapy leaders who understand the complexity of home care and are ready to coach, mentor, and support a team of clinicians across the Twin Cities. Support clinicians in providing care and patient transitions across the continuum. Home Health Care, Inc. has been serving the community for over 32 years, supporting patients across a 21-county service area. You will join a high-energy, collaborative leadership team focused on delivering best-in-class patient care.

Clinical Supervisor PT/OT Overview:

• Provide clinical leadership and supervision to nurses, therapists, and home health aides, fostering a culture of accountability, collaboration, and excellence.
• Oversee patient care plans to ensure they meet evidence-based standards and align with physician orders and regulatory requirements.
• Conduct regular case reviews, audits, and field visits to maintain high standards of care and identify opportunities for improvement.
• Mentor and develop clinical staff through coaching, training, and performance evaluations.
• Ensure compliance with all federal, state, and accreditation standards (e.g., Medicare Conditions of Participation).
• Collaborate with intake, scheduling, and administrative teams to optimize care delivery and resource utilization.
• Lead quality improvement initiatives, tracking key performance indicators such as patient outcomes, hospital readmissions, and patient satisfaction.
• Serve as a clinical resource for complex cases and escalation point for care-related concerns.
• Participate in strategic planning and contribute to agency growth and service expansion.
• Participate in on-call rotation to support clinicians and patient needs.

Clinical Supervisor PT/OT Qualifications:

• Active unencumbered Minnesota PT/OT license required.
• Prior Home Health Care experience is required.
• Strong knowledge of Medicare guidelines, OASIS, and home health regulations.
• Previous leadership, supervisory, or preceptor experience preferred.
• Experience with EMR systems and electronic clinical documentation.
• Strong clinical decision-making skills with the ability to act confidently in real time.
• Excellent communication and leadership presence with the ability to motivate teams.
• Ability to thrive in a fast-paced, team-driven environment.
