The Population Health Care Coordinator (PHCC) is responsible for advancing whole-person, population-based health (physical and mental health) management within the clinic. This role integrates care coordination for clients with co-morbid behavioral, physical, and mental health conditions, focusing on long-term engagement, health promotion, and seamless transitions across services. The PHCC collaborates with interdisciplinary teams to identify health (physical and mental health) needs, implement targeted interventions, and support clients in achieving individualized goals. Key responsibilities include client engagement, health (physical and mental health education, care coordination, data-driven population health initiatives, and supporting organizational efforts to improve health outcomes for diverse populations.
Schedule:
Full-Time. Monday-Friday, Regular clinic hours, some clinics stays open until 6pm. Locations:

Coordinate care for clients (focusing primarily on clients with co-morbid behavioral, physical, and mental health conditions), ensuring continuity, integration, and monitoring across all service domains. Coordinate and schedule the Collaborative Care Planning meeting every 6 months or as needed. Assist clients in understanding insurance renewal dates, supporting them in applying or reapplying for insurance as needed Engage clients in skill development, motivational support, and problem-solving to improve health outcomes. Provide individual and family support services, linking clients to peer and recovery support resources. Provide de-escalation for clients experiencing mental health symptoms or other challenges. Collect, analyze, and report population health data to inform program planning and quality improvement. Implement population health initiatives, including outreach, health promotion, and preventive care strategies. Facilitate evidence-based wellness and prevention education for individuals and groups, using evidence-based practices. Collaborate with interdisciplinary team members to resolve client issues, share information, and enhance engagement. Document all client interactions and population health activities in the Electronic Health Record (EHR). Required Qualifications:
Master's degree in a behavioral science or related field; OR Bachelor's degree in a behavioral science or related field with a practicum or internship that included direct work with children or adults OR 30 college credits in behavioral sciences plus one of the following: A practicum or internship with direct client interaction 2,000 hours (One year) of experience providing services to individuals with mental illness 2,000 hours (Two years) of experience working with individuals with traumatic brain injury or developmental disabilities (with additional required mental health training) OR High school diploma plus: 4,000 hours (Two years) of experience providing services to individuals with mental illness, traumatic brain injury, or developmental disabilities (training may be required 2,000 hours (One year) of experience serving individuals with mental illness, emotional disturbance, or substance use disorder with weekly clinical supervision until 4,000 hours are completed Preferred Qualifications:
Master's degree in a behavioral science or related field Bachelor's degree in a behavioral science or related field and proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields Benefits:
