Pathways Hospice and Palliative Care is seeking a dedicated and experienced Registered Nurse (RN) to join our leadership team as a Patient Care Manager (PCM) . This role is ideal for a nurse who is passionate about end-of-life care and ready to support and lead a five-star nursing team in delivering exceptional hospice services.
Position Summary :
The Patient Care Manager (PCM) provides day-to-day clinical leadership and support to hospice field staff, ensuring high-quality, patient-centered care. This role works closely with the Director of Clinical Services and interdisciplinary team to promote excellence in care delivery, compliance, and team engagement.
What You'll Do :
Supervise and support a team of hospice RNs, LPNs, and CNAs Serve as a clinical resource for patient care, symptom management, and family support Oversee daily operations including visit coordination and caseload management Ensure timely, accurate documentation and regulatory compliance Participate in and help lead Interdisciplinary Group (IDG) meetings Provide coaching, mentoring, and performance feedback to staff Assist with admissions, recertifications, and complex patient needs as needed Participate in Manager on Call rotation Why Join Pathways Hospice and Palliative Care? Requirements:
Active RN license in the state of Missouri Minimum 2 years of hospice experience Previous leadership, charge nurse, or preceptor experience strongly preferred Strong clinical judgment and communication skills Ability to thrive in a fast-paced, team-oriented environment Comfortable using electronic medical records and technology
