Non-Exempt SUMMARY The Health Coordinator supports the comprehensive health and wellness of individuals with intellectual and developmental disabilities (IDD) by coordinating medical care, facilitating communication among providers, reviewing medications and physician orders, and reporting concerns to the RN and direct support staff. This role serves as a key liaison between healthcare professionals, caregivers, and the interdisciplinary team to ensure continuity of care and timely response to health needs.
BASIC FUNCTIONS AND GENERAL RESPONSIBILITIES
Coordinate and schedule medical, dental, vision, and behavioral health appointments for individuals receiving services. Review and monitor physician orders, medication lists, and treatment plans for accuracy and compliance. Communicate relevant health updates, appointments, and provider recommendations to home staff and management. Identify and report medication discrepancies, outdated orders, or health concerns to the supervising RN. Ensure all health documentation is current and accurately entered into the electronic health record system- Therap and HRST as appropriate. Track preventive care services and routine screenings in alignment with state and organizational standards. Ensure all appointments are entered into Therap and the status is accurately updated. Provide detailed information under "results" for each appointment. Upload medical documents to their appropriate location in Therap such as the MAR, appointment consultation, and lab results. Participate in interdisciplinary team (IDT) meetings and contribute to individual service planning (ISP) meetings. Ensure new medications are checked-in accurately by checking them against the doctor's orders and the updated MAR. Prepare medication destruction documentation to dispose of discontinued, inaccurate, or contaminated medications for the nurse and as witnesses at time of destruction if available. Check the live MARs throughout the month and report discrepancies and errors to the RN. Check-in monthly cycle meds in the home. Reconfigure the next month's MAR at that time. Request medical reports and results from hospitals for complete medical records. Ensure physician's orders (POS/POF) are signed annually by the primary physician. Report any concerns or questions regarding physician orders to the nurse, pharmacy, and/or physician. Physical Expectations This position requires extended periods of walking and standing with some periods of sitting - depending on the needs and activities of the individuals supported. It may require light (5 lbs.) to heavy lifting (50 pounds of dead weight) and occasional stooping, kneeling, bending, and/or climbing stairs again depending on the needs of the individual supported. The position will require occasional physical intervention if confronted by a person displaying combative or potentially dangerous behavior. Driving an agency or personal vehicle is an expectation. Working Conditions This position will require travel between residential locations and medical offices. Must be able to sit, stand, bend, and lift moderate weight as part of documentation and client support duties. Requires flexibility and responsiveness to meet the urgent and ongoing health needs of individuals served.
QUALIFICATIONS
Must have graduated from High School or obtained a General Equivalency Diploma. Must have at least 1 year of successful employment in a work environment similar to that described in this job description, or A combination of medical education and work experience equaling 2 years may be substituted upon the approval of the Director of Nursing and Executive Director. Must obtain and maintain training and certification in all required areas such as Abuse/Neglect, First Aid, CPR, Level 1 Medication Administration, Positive Behavior Supports. Must obtain a Class E driver's license during new hire orientation- must carry minimum coverage applicable by law for auto insurance Must Receive Seasonal Flu Vaccination Equal Opportunity Employer
