Join a patient-centered healthcare team where your organizational skills and attention to detail make a direct impact on patient care. The Patient Care Coordinator is a dynamic, multi-functional role supporting medical records, referrals, patient communication, and limited clinical tasks. This position is ideal for someone who thrives in a fast-paced medical environment, values professionalism and confidentiality, and wants to contribute meaningfully to a high-quality, compliant Rural Health Clinic.
Key Responsibilities:
Manage Release of Information (ROI) processes, including handling patient and attorney medical record requests in compliance with HIPAA and clinic policies; verify authorizations, track requests, ensure timely release, and safeguard Protected Health Information (PHI) Managed patient registration processes, ensuring accurate entry of demographic and insurance information Coordinated patient check-in and check-out procedures to maintain efficient workflow and positive patient experience Performed front desk operations, including scheduling appointments, answering phones, and addressing patient inquiries Coordinate outbound referrals to specialists and ancillary providers, ensuring accuracy, completeness, and proper documentation while collaborating with patients, providers, and external offices Provide professional telephone and patient communication support by answering calls, sharing clinic information, routing inquiries appropriately, and documenting interactions accurately Perform administrative support tasks including fax processing, document management, patient access coordination, and maintaining accurate records in the software Assist with limited clinical support duties as assigned, including non-clinical patient rooming, supporting nursing staff, following infection control protocols, and documenting tasks appropriately Uphold all compliance, confidentiality, and professional standards by adhering to
HIPAA, CMS
Rural Health Clinic requirements, TCT standards, and clinic policies; maintain a respectful, calm, and patient-focused environment Address concerns professionally and report incidents with clear, factual documentation, following proper escalation protocols and participating in required training and competency validation
Qualifications:
High school diploma or equivalent required Must have experience in a medical office or healthcare setting Familiarity with electronic medical records (eClinicalWorks preferred) Strong organizational, multitasking, and communication skills Knowledge of HIPAA regulations and patient confidentiality standards Ability to work efficiently in a fast-paced environment with strong attention to detail
