Under the direction of the Program Manager, the incumbent is responsible for the strategic coordination, oversight, monitoring, and continuous improvement of healthcare-related re-entry services within the City Justice Center (CJC). The position ensures that re-entry programming aligns with evidence-based practices, accreditation standards, and Department of Health priorities while supporting successful community reintegration outcomes.
The incumbent serves as a key liaison between healthcare providers, Department of Health leadership, corrections personnel, and community-based partners, ensuring effective collaboration, accountability, and operational alignment of re-entry services.
Essential Functions and Responsibilities:
- Provides oversight, assessment, and evaluation of the planning, organization, and coordination of CJC healthcare re-entry services, including but not limited to employment, housing, education, substance use treatment, behavioral health services, discharge planning and continuity of medical care.
- Monitors program performance through outcome tracking, data analysis, and evaluation of provider effectiveness in supporting successful reintegration and reducing recidivism.
- Develops, maintains, and analyzes program metrics; compiles data and prepares comprehensive reports documenting service delivery, outcomes, compliance, and recommended improvements.
- Collaborates with interdisciplinary healthcare and Corrections teams to address re-entry barriers affecting individual residents and system-wide populations.
- Supports implementation of continuous quality improvement (CQI) initiatives by integrating performance evaluation, process improvement strategies, and collaborative care models into program operations.
- Assists in ensuring compliance with quality assurance requirements, including adherence to current National Commission on Correctional Health Care (NCCHC) standards and accreditation expectations.
- Participates actively in case conferences, multidisciplinary meetings, and operational planning sessions.
- Establishes and maintains productive working relationships with community healthcare and human service providers engaged in the re-entry continuum of care.
- Serves as a Department of Health liaison and subject matter resource for healthcare staff, corrections personnel, community organizations, and local, state, and federal partners.
- Coordinates with community partners to support transition planning and obtain post-release outcome data for program evaluation.
- Represents the program at interagency meetings, professional conferences, trainings, and workshops related to correctional healthcare and re-entry services.
- Performs other duties as assigned.
Knowledge, Skills and Abilities:
- Working knowledge of accreditation standards, regulatory requirements, and evidence-based practices related to correctional healthcare and re-entry services.
- Demonstrated proficiency with electronic health record (EHR) systems and healthcare data management platforms.
- Strong analytical skills with the ability to interpret data, identify trends, and translate findings into actionable program improvements.
- Ability to maintain strict confidentiality while exercising sound professional judgment in a high-security and sensitive environment.
- Proven ability to plan, implement, monitor, and evaluate programs or services with measurable outcomes.
- Excellent written and verbal communication skills, including professional report writing and formal presentations.
- Advanced proficiency in Microsoft Word, Excel, and PowerPoint, including data tracking and reporting functions.
- Ability to conduct intermediate-level data analysis and performance monitoring.
- Strong organizational skills with the ability to manage multiple priorities, deadlines, and stakeholders simultaneously.
- Demonstrated professionalism, accountability, and a service-oriented approach.
- Ability to work effectively within interdisciplinary teams, demonstrating collaboration, initiative, emotional intelligence, and resilience.
- Working knowledge of City of St.
Louis policies and procedures or the ability to quickly acquire and apply such knowledge.
Qualifications
An Associate's degree in Public Health, Healthcare Administration, Criminal Justice, Public Administration, Social Work, or a related field; plus two years of work experience in a healthcare, public health, social services, case management, or re-entry programming setting. OR an equivalent combination of education, training and experience.
Preference:
Experience working in correctional healthcare, forensic healthcare, or justice-involved populations.
License:
Must possess and maintain a valid driver's license while employed by the City of St. Louis. Must possess a valid driver's license at the time of filing application and be able to present it upon request. City Residence Preference Points
City residents who pass an examination for a position in the classified service and do not receive any other preference will receive a five (5) point preference on a scale of 100 points on the exam. An additional one (1) point shall be added to the passing score of City residents who receive any other preference on a scale of 100 points.
