Job Description
The Patient Clinical Manager is a Registered Nurse who is a member of the patient care team within the Department of Home Health and is directly accountable to the Clinical Director. The position requires that the individual be clinically competent, demonstrate leadership abilities, and be knowledgeable in home health regulations. Responsible for the coordination and supervision of services, making patient and personnel assignment, and coordinating patient care and referrals. Responsible for assuring patient needs are continually assessed and development, implementation, and updates of the individualized plan of care. Assures quality of services, appropriate staffing, and all services available. Follows up on timely on any patient or customer complaints. Responsible for daily and ongoing workqueues and reports working and correcting any issues and problems as related to clinical and account deficiencies. Responsible for monitoring of office daily reports for physician orders, admissions, discharges, encounters and working with staff on timely and efficient documentation. Responsible for follow-up and investigation of all orders entered by clinicians insuring accuracy of order, necessity of order, and appropriate provider selection. Demonstrates the ability to manage office expenses related to supplies, medications and other expenses as applicable. Responsible for the department quality measures, process measures, patient and staff satisfaction within their office and overall agency. Responsible for managing employee time and productivity. Performs other job duties as assigned. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed
Performance Expectations:
Demonstrates ability to coordinate all patient care services. Ability to schedule patients using EMR. Monitors reports and develops proficiencies working and monitoring work ques to facilitate the patient's medical record and ensures claims are submitted timely. Ability to arrange medications, supplies, and equipment as ordered. Demonstrates ability to manage staff productivity and ensuring staff scheduling of patients and proper documentation according to agency policy.Qualifications:
Education/Skills Graduate of a recognized non-online RN program. Bachelor of Science in nursing preferred, minimum of ADN required.Work Experience:
Two years of experience in home health nursing required. One year of supervision experience required; knowledge of state and federal home health regulations preferred. Certification/Licensure-DUEUPON HIRE
Licensed RN able to practice within the State of MS Valid Driver's License and proof of automobile insurance required. Additional Certification/Licensure - Obtained based on required timeframe below Basic Life Support Within 30Days of Employment Required Mental Demands:
