Stanly Adult Care Center, Inc. is a nonprofit Adult Day Health program serving adults with chronic health conditions, physical disabilities, mild cognitive impairment, and age-related challenges. We are seeking a compassionate, organized, and dedicated Registered Nurse to serve as our Healthcare Coordinator. This is a unique opportunity to help shape a growing program that focuses on dignity, independence, meaningful engagement, and caregiver support. Position Summary The Healthcare Coordinator (RN) oversees the healthcare needs of participants, coordinates nursing services, supervises direct care staff, and works collaboratively with the interdisciplinary team to promote participant health, safety, and quality of life. Responsibilities Conduct admission and ongoing nursing assessments. Develop, implement, and review individualized care plans. Administer medications and treatments as prescribed. Monitor participant health status and communicate changes to families and healthcare providers. Maintain accurate medical documentation and ensure HIPAA compliance. Supervise and support nursing and personal care staff. Provide staff education and training on healthcare-related topics and emergency procedures. Participate in participant admissions, care conferences, and quality assurance activities. Assist with personal care activities when needed. Support the mission and daily operations of the center. Qualifications Current North Carolina Registered Nurse (RN) license in good standing. Minimum of two years of nursing experience with older adults, adult day health, geriatrics, rehabilitation, home health, long-term care, or related settings. Strong leadership, communication, and organizational skills. CPR and First Aid certification. Ability to work effectively as part of a multidisciplinary team. Compassion for serving adults with chronic conditions and age-related challenges. Physical Requirements Ability to stand and walk for extended periods. Ability to lift, push, or pull up to 50 pounds.
